I found this one easiest to be done at the beginning of Chapter 6, when you become stranded. Go over to the right hand side from your start point to find grenades and a shotgun. Proceed down this hallway, and you should have just flanked 4 enemies, with a shotgun in your hands at a very close range. Kill them all to unlock the achievement. This will work on any difficulty. If you are having trouble using the above method, just keep at it, and use the shotgun in close quarter combat situations when enemies are running towards you, and you can always try to try the difficulty down.
